I have a PCI-6040E that appears to be unable to measure or generate analog signals. I ran the self test application at www.ni.com/support/selftest and the tests of the analog input and output channels failed and the card failed its self-calibration every time I tried to run it. I have included the Pass/Fail summary below and the portion of the test results that I was able to copy are attached, but I was unable to submit the results to NI after the tests were complete. What is my next step? Send the card to NI for repairs?

Please contact your local NI Office directly with that. I think you might need to do a repair or at least a calibration.
