What should I do if my card does not pass the tests for AI and AO functionality?

I have a PCI-6040E that appears to be unable to measure or generate analog signals. I ran the self test application at www.ni.com/support/selftest and the tests of the analog input and output channels failed and the card failed its self-calibration every time I tried to run it. I have included the Pass/Fail summary below and the portion of the test results that I was able to copy are attached, but I was unable to submit the results to NI after the tests were complete. What is my next step? Send the card to NI for repairs?

PASS: Detection -- Device Detection and Resource

PASS: Single Pulse -- Single Shot Pulse Width Verification

PASS: Continuous Pulse Train -- Buffered Pulse Width Verification

FAIL: Calibration -- Analog Input Calibration

FAIL: Calibration -- Analog Input Calibration

Please contact your local NI Office directly with that. I think you might need to do a repair or at least a calibration.
