You have a few options here:

Look specifically at the Model Plugin ones.
2- another option is to customize the stylesheet. http://digital.ni.com/public.nsf/allkb/EC64D7C24421A2E58625751D00491809 There are tools on www3Schools.com that show you how to use stylesheets.
Again example in the same folder above.
What format do you need it in? Word? If it's Word then go with option 1.
